• Nurse Care Manager/Care Coordinator Standardized Core Curriculum (GLearn) Training Program Application

  • The Care Transformation Collaborative of Rhode Island with funding from UnitedHealthcare is pleased to offer up to ten (10) Nurse Care Managers/Care Coordinators the opportunity to participate in a standardized evidence-based training program in which you will learn to apply key care management concepts within your primary care practice setting and organization.

    A complete application includes the following form containing questions about your contact information, practice contact in- formation, license information, job function, specialty, etc.

    Who Can Apply?
    Nurse Care managers and Care Coordinators working in Rhode Island primary care practices (pediatric, adult and family medicine). If more than 10 NCM/CC apply for the training, priority will be given to applicants whose primary care practice is part of a system of care (accountable care organization) and/or participated in the CTC-RI transformation program. Applicants may be reached out to for clarity on their role and program fit.

  • NCM/CC GLearn Core Curriculum Training Application Agreement

  • GLearn Requirements:

    • GLearn™ care management online training modules are the intellectual property of GLearn and therefore may only be used by the individual authorized/licensed by CTC to participate in CTC-sponsored GLearn care management training Licensees will not use, copy, perform, display, modify, create derivative works, merge, distribute otherwise exploit the GLearn™ care management training modules.
    • I have read the above and agree to abide by the stipulations agreed to in the contract between CTC-RI and GLearn™.
    • I have spoken with practice leadership and understand the time commitment including weekly meetings, time complete modules and self-study time.
    • I agree that I will direct any questions I have to CTC leadership who are most knowledgeable about the GLearn contractual agreement. A copy of the contract is available upon request.
    • Failure to complete the course and/or maintain communication with CTC-RI program staff may result in the learner being required to reimburse course costs, up to a maximum of $3,500.
    • I understand that each module includes a required evaluation, which must be completed for the module to be deemed complete and for CEUs/CMEs to be issued.
