  • Asthma Core Training for CHW & MA Program Application

  • The Care Transformation Collaborative of Rhode Island (CTC-RI), in partnership with the Rhode Island Department of Health (RIDOH), is excited to offer up to 15 Community Health Workers the opportunity to participate in the Asthma Basics Training Program. This comprehensive program spans two days in person, totaling 9 hours with a virtual assessment via zoom (to be scheduled on the last day of class). It is designed to enhance participants’ skills in asthma education, empowering them to better support patients and help reduce emergency department and hospital visits. Please note that completion of this training is a prerequisite for the upcoming Asthma Home Visiting Training.

    Goal:
    The goal of the Asthma Basics Training Program is to prepare entry-level community health workers (CHW) or Medical Assistants to effectively provide basic asthma management education.


    Format:
    Training course will take place on over the course of two days in person on Feb 4 & Feb 6, 2026. Additional Dates are listed for future sessions.

  • Community Health Worker/Patient Care Navigator(Participant) Roles and Responsibilities

  • The Asthma Education for Community Health Worker Training program has been financially supported by the RI Department of Health through funding from the Centers for Disease Control and Prevention (CDC).

    Based on adult learning research, each module is created and taught separately allowing participants adequate time to review and assimilate the content. During each module, learners will participate in multiple active learning activities to work with new content and review previously taught subject matter. The trainer uses multiple tools to assess each learner’s comprehension of the content through the training, allowing opportunities to work with them individually to enhance understanding and proficiency at explaining the material appropriately to others.

    With this in mind, applicants need to commit to meeting the learner responsibilities at outlined below:

    • Attending all training dates for the whole sessions ( modules and evaluation at conclusion of training).
    • Obtaining the support of organization supervisor/leadership and ensuring adequate time to attend training.
    • Participating wholly in training including discussion, assignments and practice activities.
    • Seek clarification and assistance when needed to fully understand the material.
    • Demonstrate respect and professionalism towards trainers, fellow participants, and community members.

    I understand that this program will be over the course of two days plus an assessment, totaling approximately 10 hours I can allot the time needed to complete this program and assessment.

    I understand that failure to attend all modules and complete evaluation will result in an incomplete in the course and participant will not be eligible for certificate or any stipends.

    I have discussed my intent to attend the Asthma Education for the Community Health Worker Training Program with my supervisor who has indicated support for me to attend and meet the participation responsibilities or indicate not applicable.
