Join Care Transformation Collaborative of RI on Thursday, November 19th, at the Crowne Plaza in Warwick Rhode Island for “Advancing Primary Care: Envisioning What’s Possible”.

This in-person event promises enriching learning opportunities across key topics such as: health equity, population health, best practices in behavioral health (IBH), technology, addressing workforce shortage, improving child/family health and more! As we delve into these critical areas, attendees can expect to gain invaluable insights and strategies to improve patient outcomes by leveraging the strengths of the team, the system of care and patient voice.

In addition to our engaging educational sessions, our conference will feature an exciting exhibitor hall, showcasing community-based organizations, partners, and health care agencies and their services. Attendees will have the opportunity to explore and meet with a diverse range of exhibitors, network, and access cutting-edge industry and local resources. Keynote Speaker

Our Keynote speaker this year is the founder of MAPCAP (MA Primary Care Alliance for Patients), Dr. Wayne Altman.

Target Audience

All CTC-RI practices, learning collaborative participants and partners, past and present, are welcome to attend. We invite practice leadership, providers, nurses, nurse care managers, care coordinators, behavioral health staff, practice managers, pharmacists, medical assistants, community health workers, and CTC-RI stakeholders to come learn as a team!

Registration Information

Registration will begin at 8:00am EST with a light breakfast. Our welcome address will begin at 9:00am.

CME Accreditation

CME’s/CEU’s and certificates of attendance will be available after the conference. CHW certificates of attendance will also be available.
